The membership fee you pay entitles you to most in-office services at no additional charge (listed as INCLUDED below). There are small charges (still at large discounts) for a few things that cost the practice a bit more to provide.

Membership Fees + Discounts 

Base rates for a month of membership are shown in the table below.

Rates are set by patient's age, as needs vary most by age in primary care. There are pro-rated rates based on when you sign up in a month.

Discounts

Discounted rates are available for the following:

Families: 10% discount
A "family" is three or more individuals from the same household/same physical address

Advance Payments:
Paying three months at a time: 3% discount
Paying 6 months at a time: 6% discount
Paying annually (12 months at a time): 12% discount

Discounts can be added together (if a family pays six months in advance, the total discount is 16%).

 

Terminating/Suspending a Membership

  • Terminating and re-enrolling

    • If you terminate a membership for more than three months, and then wish to resume care, there will be a re-enrollment fee ($200 for the first person, and $100 for each additional person on the contract).

    • Hardships (economic and otherwise) may waive this fee if discussed with Dr. LoPresti in advance of the end of your membership.

  • Suspending a Membership

    • If you routinely travel to another place for extended periods (such as going away to college or south for the winter), arrangements can be made to suspend your membership, but not terminate it, to avoid the re-enrollment fee. You must notify us at least one month in advance and pay $25 for each month the membership will be suspended. The entire suspension period must be paid in advance.
